i know paintsville and prestonsburg used to be big until the gap between paintsville and pbrug athletic wise got wayyyyyy wider.

Paintsville\Johnson Central just because they are located in the same city and have been playing each other so long. It also helps that they both usually have very competitive teams.

I would go with Paintsville and JC only because they have been playing each other for so long. Shelby Valley and pikeville is second but not by much.

Paintsville/Central and it's not even close.

Best rivalry in the region, arguably one of, if not the best in the state.

What makes it so great is just the emotion, the fact that they're separated by just a few miles, and if one team is having a down season and the other is having a great season, it's still usually a close contested game.
