Why do these things keep happening? Why can't we learn from our past mistakes. John M. Deutch a former CIA director and Deputy Secretary of Defense said "These screw-ups happen".
"WASHINGTON (AP) - The government accidentally posted on the Internet a list of all civilian nuclear sites and their activities in the United States.
The 266-page document was published on May 6 as a transmission from President Barack Obama to the U.S. Congress. According to the document, the list was required by law and will be provided to the International Atomic Energy Agency.
Some of the pages are marked "highly confidential safeguards sensitive." While there is security at the facilities, the list could presumably be useful for terrorists or anyone else who would like to harm the United States."

And then from MSNBC News:
"The federal government mistakenly made public a 266-page report, its pages marked âhighly confidential,â that gives detailed information about hundreds of the nationâs civilian nuclear sites and programs, including maps showing the precise locations of stockpiles of fuel for nuclear weapons."
